Intensive Case Manager

The Case Manager assists clients with substance use disorder in gaining access to community services and agencies. This linkage will assist those in recovery with their efforts to live in the community in a safe, stable, and healthy environment that promotes a drug and alcohol free lifestyle. The Case Manager is responsible for assisting clients in setting goals, accessing services to meet those goals, and serving as advocates to clients in their recovery journey. Case Managers assist in facilitating treatment to address clients problems relative to alcohol and other drug usage

Seeking candidates that are self-starters, work well independently, attention to detail and documentation. Maintain a caseload and collaborate interdepartmentally. Maintain working knowledge of Delaware County community resources.

Requirements:
Bachelor's degree with major course work in sociology, social work, psychology, gerontology, anthropology, criminal justice, theology, nursing, counseling or education.

Valid drivers license.

One year of drug and alcohol work experience and motivational interviewing experience preferred.
