Job Description

Position Description:

The goal of the Be You Program is to increase youth optimal health, increase students knowledge of the eight dimensions of wellness, provide opportunities for positive youth development and focus on adulthood preparation subjects that will support their decision and goal setting for their future. The program focuses on increasing youth agency, senses of value, purpose, legacy and their decision making skills for a positive future. Facilitators will implement evidence based comprehensive health curriculums in the school setting and community outreach. Medically accurate information on STD, HIV and birth control are emphasised to curb teen pregnancy rates. In addition, we aim at connecting youth to integrated primary care including behavioral health and family planning services for STD testing and pregnancy test.

TA Outreach Facilitator will ensure high-quality fidelity of implementations of evidence based teen pregnancy prevention curriculums in the school setting. Facilitators will also perform observations with the participating teachers in school-based settings and implement curriculum as needed. Will report on all operational and administration duties, implement program activities, implement components of the community mobilization, support department activity though community outreach events and other duties assigned by the supervisor.

Roles and Responsibilities
  • Fidelity of implementations: Ensure evidence based curriculums are being implemented with fidelity by performing observations.
  • Be in constant communication with the Fidelity Specialist to identify details in order to perform observations.
  • Collect fidelity data including but not limited to implementation dates, teacher fidelity logs.
  • Submit data to the corresponding data collection database or software.
  • Provide on-going information, referrals, linkages, and advocacy for all other identified needs.
  • Health Fairs/Community Events Participate in health fairs/community events throughout the community and/or areas surrounding the PVHC clinics. * Sponsor, organize, and/or host health fairs with partners to help the neighboring community become familiar with PV/PVCDC/PVHC.
  • Presentations PV/PVCDC/PVHC services presentations at different venues to disseminate information regarding services.
  • Seasonal Outreach Door-to-door neighborhood canvassing distributing flyers of PV Services (i.e. back to school, flu shot campaigning, Open Enrollment period).
  • Social Media Outreach Will generate social media outreach material and disseminate following PVHC policies and procedures for social media platform management.
  • Education: Educate, increase awareness, reduce stigma, and empower the community to promote behavior change through best practices and evidence-based initiatives. Provide education and resources to promote individuals and communitys self-sufficiency
  • Promote health education by implementing best practices and evidence based programs with fidelity
  • Educational encounters are facilitated on a one-to-one basis, and in group settings.
  • Educational materials are culturally competent and tailored to the needs of the community
  • Educate to build capacity of partners and stakeholders
  • Educate to help break the stigma associated with different health conditions
  • Navigation:

Eliminate barriers to access and link clients to health and social services Follow the Navigation Work Flow to access clients to integrated primary health services, interventions, treatments and telehealth services. Collaboration: Create a broader networking alliance with agencies in order to collaborate and expand opportunities to access clients to health and social services.Establish a referral linkage with partner agencies and maintain the relationship*
  • Actively participate in coalitions or community mobilization effortsEducation Requirements
  • Community Health Worker Certification or BA/BS Degree in Education, Public Health, Health Promotion, Psychology, Sociology, Anthropology and/or one-three year equivalent job experience working with youth, schools or non-profit sector
  • Bilingual (English/Spanish)
  • Culturally competent in border culture to provide culturally appropriate education and information to a broad range of audiences.
  • Computer, internet and analytical skills are required. (e.g. technical proficiency in Microsoft PowerPoint, Excel, Word, and Google Docs)
  • Data management system for the collect data for evaluation and continuous quality improvement through data collection tools and strategies
  • Report on program performance measures as needed for evaluation requirements program performance measures

  • Strong organizational and professional skills (e.g. attention to detail, balance priorities and time management)
  • Must possess strong interpersonal and relationship building skills
  • Strong public speaking skills with small and/or large groups

Additional Requirements:
  • Must have a reliable vehicle with current liability insurance and a valid drivers license.
  • Ability to lift and carry items up to 25 lbs.
  • Ability to follow ethical and legal standards (e.g. HIPAA)
  • Must be able to work non-traditional hours and weekends on occasions.
  • Must be self-directed and require limited oversight in performance of duties.
  • Strong written and verbal communication, listening, and public speaking skills; must be comfortable presenting to large audiences
