The Department of Human Services - Glenwood Resource Center is looking to fill a Qualified Intellectual Disabilities Professional (Treatment Program Manager ) position to assist the state. SCHEDULE: Variable schedule of hours per week to include AM/PM shifts, weekends and holidays SPECIAL REQUIREMENTS: Per CMS Regulations, 1 year of experience working directly with persons with intellectual disabilities or other developmental disabilities; and is one of the following: A doctor of medicine osteopathy, a registered nurse or an individual who holds at least a bachelor's degree in occupational therapy, occupational therapy assistant or physical therapist. JOB DUTIES: Develop and coordinate the implementation of person centered Individual Support Plans (ISP) which integrates all services, supports, treatment plans, interventions, and strategies for overcoming identified barriers to live in the most integrated setting for the individuals assigned to your caseload. Assure supports are developed based on comprehensive professional assessments and written so they may be easily understood by staff responsible for implementation. Completed documents shall be written to meet best practice standards, are Competency Based Trained (CBT) to all staff responsible for their implementation, and implemented to assist each individual exercise self-determination and independence, reflecting the desired lifestyle of the individuals. Conduct Interdisciplinary Treatment team meetings (IDT), as needed and a minimum of monthly, involving the discussion of the individuals assigned to your caseload, documenting the coordination, integration, and review of all aspects of the individual's treatment through the Monthly Integrated Review (MIR) process/protocol. Monitor and evaluate the development/implementation of goals and objectives to assure they address identified priority skill areas from the ISP; the person receives a continuous active treatment program consisting of needed interventions and services in sufficient number and frequency to support the achievement of the objectives identified in the individual program plan Identify, track, and provide appropriate recommendations/summations of all incidents directly involving the care, treatment or habilitation of individuals assigned to your caseload through implementation of the Incident Management Policy. Assure all incidents are monitored, reviewed, addressed by the interdisciplinary team according to policy standards and documented within the time frames specified. Supervise and evaluate treatment team personnel assigned to your leadership. Recommend personnel actions related to selection, performance, work schedules, work assignments, disciplinary actions to be taken, and implementation of policies, procedures, and protocols developed for Glenwood Resource Center (GRC) and Department of Human Services (DHS). Provide quality care as confirmed by Quality Indicators in your treatment team area. Proactively promote and participate in Quality Assurance efforts designed to improve the quality of life and services of people who you provide services to. Document and monitor Quality Indicator data for individuals assigned to your caseload. Assist direct support staff in understanding how this data relates to the people they provide services to and how the data can be used to improve services. Coordinate competency based staff training (CBT) to assure personnel assigned under your supervision are trained and re-trained, as appropriate, on DHS and GRC policies/procedures/protocols, standards of practice identified by regulatory agencies, treatment plans to be implemented by staff related to the Individual Support Plans (ISP), Behavior Support Plans (BSP), Individualized Implementation Programs (IIP), Physical and Nutritional Management Plans (PNMP)/justifications, and corrective action plans. Provide supervision during meals, work/school. Complete and document mealtime, Physical Nutritional Management Plan (PNMP) and Behavior Support Plan (BSP) compliance monitors identified according to established schedules and policies, as well as active treatment and environmental audits. Graduation from an accredited college or university with a bachelor's or master's degree in a professional discipline related to provision of human care and treatment, a human service field, a field of study related to basic human care needs, human behavior, human skill development, humans and their cultural behavior, the human condition, or any other study related to basic human care needs and experience equal to one year of full time work with persons with intellectual disabilities or developmental disabilities; OR licensure as a doctor of medicine or osteopathy or as a registered nurse and experience equal to one year of full-time work with persons with intellectual disabilities or developmental disabilities. For additional information, please click on this link to view the job description. * Licensure or temporary licensure by the State of Iowa is required where applicable under state law (occupational therapy, physical therapy, audiology, speech pathology, doctor of medicine or osteopathy and registered nurse). * Among the acceptable degrees: occupational therapy, physical therapy, speech pathology, audiology, sociology, education, rehabilitation counseling, social work, psychology, speech, communication, gerontology, counseling, human development, recreation, special education, anthropology, literature, art, dance, and music. Experience is applicable to work before or after the required degree. * At the time of interview, applicants referred to Glenwood and Woodward State Hospital-Schools will be assessed to determine if they meet federal employment requirements as published in the Federal Register, Section 20 CFR 483.430. * Designated positions in this job class require applicants to obtain the required Commercial Drivers License and endorsements within a period of time as determined by the appointing authority at the time of hire.
