QUALIFICATIONS:
  • Bachelor's Degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, criminal justice, criminology, theology, nursing, counseling, education or other related social science or human services field.
  • One to three years working with families or individuals in a mental health setting is required.
  • One to three years case management experience required.

  • REQUIREMENTS:
  • Must possess excellent verbal and written communication skills.
  • Must have a valid driver's license, acceptable driving record, and access to a reliable automobile.
  • Ability to adapt hours to needs of position. May need to work evenings and/or weekends hours periodically including providing on-call emergency services on a rotating basis.
  • Access to a telephone.
  • Ability to remain organized despite pressured situations.
  • Must be eligible for PA State Police clearance, PA Child Abuse History Clearance, FBI Clearance, Arrest/Conviction Report and Certification Form, and Act 168 Forms.
  • Must be able to identify with the agency's Mission Statement and support the agency's Core Values.

    ANNUAL REQUIREMENTS:
    • Training: Family Advocates are required to complete designated trainings related to their job performance annually.
    • Medical/Physical: Family Advocates are required to obtain a post-offer TB test and drug screening.

    CORE COMPETENCIES:
  • Obtain and complete the service history information, initial assessment material, encounter forms, service documents, and the individualized service plan in a timely and accurate manner.
  • Assist consumers with accessing appropriate community services, living in the least restrictive setting, obtaining basic necessities, and conducting activities of daily living.
  • Provide on-call services as assigned and develop crisis plans for assigned consumers.
  • Ensure the supervisor is informed regarding consumers, as well as accepting and utilizing routine supervision.
  • Develop and maintain professional relationships in the community and with other office staff.
  • Provide services at the level indicated by the consumer's individualized service plan and outlined by the pertinent regulations.
  • Complete discharge documentation in a timely and accurate manner.
  • Liaison with County CYS to accept referrals and facilitate agency admissions, transfers and discharges.
  • Keep the Director of Family Based MHS and/or FWP Coordinator/OP supervisor informed of high-risk cases, pertinent issues related to clients and contracting agencies.
  • Carry a full caseload, up to four families, and provide direct care in accordance with the following requirements; Develop Service Plans in collaboration with families, county and community representatives. Complete Service Plans in a timely fashion in accordance with regulations and agency policies. Involve family members and stakeholders in the service planning process and indicate participation through obtaining signatures of all participants.
  • Complete a psychosocial history for all clients within 30 days of admission to Glade Run Lutheran Services. Conduct interagency meetings as needed or required by agency's policy. Access and link families to needed community resources.
  • Provide discharge planning and follow-up, beginning with planning at the time of admission. Submit required information for program database in a timely and thorough manner.
  • Complete all required paperwork recognized in the designated record set within 30 days of admission.
  • Will work collaboratively with families in order to provide families with needed support and education related to their individualized needs including childcare and transportation.
  • Will work cooperatively and in conjunction with all Family Advocate/Case Managers to ensure the needs of the families are met.
