PURPOSE

Provides a range of direct and indirect services to a blended caseload of adults with serious and persistent mental illness who are at various stages of recovery. These adults require assistance in accessing, coordinating and monitoring resources and services in order to live as independently as possible in the community.

FUNCTIONS

1. Assumes direct responsibility for a caseload of 17-29 People in Recovery (PIRs). This includes but is not limited to:

a) Engages PIR in an ongoing supportive relationship.

b) Provides aggressive outreach and follow-up services in PIR's homes, hospitals, programs and other community settings.

c) Provides clinical case management services to PIR including assessment, planning, monitoring, linkage and advocacy services.

d) Educates PIR as to the nature and effects of their illness, including medication effects, current treatment techniques, and current research developments.

e) Assists PIR in defining their strengths, needs and interests.

f) Assists PIR in assessing and developing skills.

g) Assists PIR in developing a Personal Goal Plan based upon assessed strengths, interests and needs.

h) Assists PIR in accessing services for the purposes outlined in

their plans, including advocacy on their behalf.

i) Monitors and coordinates the provision of services to PIR.

j) Coordinates plans with significant others in PIR's support system.

k) Assists PIR, as needed, in developing basic living skills, e.g. self-care, mobility, housekeeping, budgeting, etc.

l) Maintains required records and documentation as required by Agency, County, State and Managed Health Care and/or as requested by supervisor.

m) Provides 24 hour services to PIR on a rotating basis.

2. Ensures immediate follow-up when PIR enter the Crisis Service.

3. Provides liaison service with hospital staff when PIRs are hospitalized.

4. Participates in daily team meetings and TCM administrative meetings.

5. Must attend all mandatory yearly trainings and additionally receive 10 hours of elective trainings per year as approved/recommended by supervisor.

6. Other job related duties as assigned.

7. Adheres to safe work practices.

8. Must function in full compliance with all applicable policies and procedures, federal state and local laws and Interact's Code of Conduct. Must participate in Interact's Compliance Program including reporting suspected violations of the compliance program or applicable laws or regulations to Supervisor/Manager/Director or the Compliance Officer.

9. Must participate in Interact's Quality Improvement activities as requested by Supervisor/Manager/Director or Quality Assurance Department.

Job Requirements:

QUALIFICATIONS

1. A bachelor's degree with concentration in sociology, social work, psychology,

gerontology, anthropology, criminal justice, theology, nursing, counseling or education or a bachelor's degree in any field plus a minimum of 12 college credit hours in sociology, social welfare, psychology, gerontology, or other social science plus 1year of mental health direct care experience.

2. Knowledge of psychiatric disabilities, current treatment approaches and

current research developments.

3. Good communication, interpersonal and organizational skills.

4. Ability to work as part of a team.

5. Availability of an insured vehicle, valid driver's license from state of residence; a minimum of one (1) year driving experience; a driving record free of at-fault accidents and/or moving violations for the past year.

6. Ability to lift/carry thirty (30) pounds.

7. Ability to share office space and work in a group setting.

8. Clearance in accordance with PA Acts 73, 33 & 80.
